A current SPORTS PHYSICAL. Sports Physicals are valid for three years. (Unless stated otherwise by a physician)
A completed MSHSL Eligibility Form, which is valid for the current school year.
Participation Fee for Golf would be $70
For those who qualify for free/reduced lunch the fee for each sport is $20.
