To participate in athletics at Johnson High School you need to turn in the following to the Athletic Office:

• A current SPORTS PHYSICAL. Sports Physicals are valid for three years. (Unless stated otherwise by a physician)
• A completed MSHSL Eligibility Form, which is valid for the current school year.
  For those who qualify for free/reduced lunch the fee for each sport is $30.
