To participate in Basketball at Johnson High School you need to turn in the following to the Athletics Office:

• A current SPORTS PHYSICAL. Sports Physicals are valid for three years. (Unless stated otherwise by a physician)
• A completed MSHSL Eligibility Form, which is valid for the current school year.
  All forms can be found at Athletic Forms and Information on the Quick Link tab
• Participation Fee for each sport are $60 ($30 free or reduced lunch.)
